Out of a grapple? Yes. Teleportation of any sort would count as "an effect removes the grappled creature from the reach of the grappler". Out of chains? Yes probably, though any dungeon worth its salt would either fully bind and gag a spellcaster, or keep a few sets of magical "no you can't teleport out of these" shackles on hand.
From reading the description of the Teleport spell, the caster wouldn't have to grab the prisoner - the prisoner would just need to be within the 10ft area of the spell.
The spell doesn't discuss restrictions on what "equipment" any creature that is teleported brings with them, so I think we can safely assume that they teleport with whatever they are carrying.
If the prisoner is chained to a wall, then I expect that the chains remain anchored to the wall and don't teleport with the prisoner, though it's up to the DM, which allows for interesting story-telling, like having the wrist and leg cuffs still attached to the prisoner, with short lengths of chain trailing off them.
Can you Misty Step or Teleport out of a grapple?
Can you grab somebody in chains and Teleport them away out of the chains?

What I meant was if there was a prisoner in chains and the caster wanted to free them, could the caster grab the prisoner and t-port out.

Yes, as long as the spell lets you bring a passenger and isn't gate based. It is up to DM discretion in most cases.
