What moment as a player or as the observing DM were you like "well crap, we're dead"? The outcome might be good or TPK but I just wanna laugh at the insanity of these moments.

Hello There. I am a worldbuilder and proud DM that is creating a huge world called Eldredom. I'm pouring many hours into it and I may make some things later...
Literally the first encounter. All new players. Not a single one had played before. When one of them died to poison from one of the giant wolf spiders, I was like "Welp, were done."
Party of 6 L19/20 characters vs Lolth in the Demonweb Pits and I go last in initiative. The party goes and annoys her, she goes and tpks the rest of the party, no one else around so I’m like ok I’m dead, can I get her to kill me quickly? Amazingly I survived.
There was this one fight ... we weren't that high level, and we were going to fight a big dragon. The GM has not really made any secret of this dragons +1000 years of lifespan, so we knew ... this wasn't really a fight we had any reason to be confident about.
So we work our way into the lair. Being careful, but still expending ressources. And then we reach the hoard, but the dragon isn't there. What is there, is a backdoor, robustly locked, with magic. No way out.
At which point of course we realise the dragon is behind us.
Oh *expletive*, we're *expletive*'d.
We lived, by the way. When you're forced to actually pull out all the stops to survive, characters are just surprisingly ressourceful =)

As far as I recall, that wasn't even it - I, for one, had a run of truly awful rolls. In the end it was more a case of digging deep in our pockets and throwing whatever was in there at the dragon. Like, I was a bard. I had a Canaith Mandolin. I rarely actually used it for spell casting, but this fight, I used everything in it. We gobbled every potion, spent every charge, and so on.
Maybe it's just my group, but I feel stuff often just drops into the bag of holding, and is forgotten. Another time, we got caught in a dungeon by a mummy lord. It cast Creeping Doom on us, in a place we simply couldn't escape it. We were *bleeped*. Only, at the bottom of my pack I had a ring of shooting stars I'd never used before, and frankly didn't know I had. Well, in the dark, underground, Mummies aren't super cool with having massive fire damage launched at them.

I was thinking more of my escape from certain doom. Me (ranger 17/monk3) vs Lolth. Back in 2e days - made a called shot (eye shot) with an arrow of slaying I’d been saving for an emergency. Rolled a 20. DM called for godly resistance - rolled an 01. He tried everything he could think of at 1-5% chances and I rolled it for like 8-10 rolls in a row. So Lolth died and I survived.
I was thinking more of my escape from certain doom. Me (ranger 17/monk3) vs Lolth. Back in 2e days - made a called shot (eye shot) with an arrow of slaying I’d been saving for an emergency. Rolled a 20. DM called for godly resistance - rolled an 01. He tried everything he could think of at 1-5% chances and I rolled it for like 8-10 rolls in a row. So Lolth died and I survived.
That is .. pretty crazy. I've had similar occasionally. I remember rolling 0001 on a D1000, which for that system was pretty good, because it was a prayer roll, and on a 1 god would literally intervene on your behalf. So that was kind of a show stopper =)
I've also once rolled 3 consecutive natural 20's. But not for anything really spectacular. The D1000 above was also a crisis, but not ... a TPK, or any such thing. I think we were escaping a prison or something.
Over 30 or 40 years of rolling dice, I guess the outliers come up occasionally =)
