Then it could still be anything.It could be Tiamat, it could be a random old one-y creature. It could be Torog or pretty much anyone else. It's like showing us a blurry picture of a random person and ask us what they work with. We can't know because we don't have anything to go on.

I would also say however, that Lilly’s patron only ever appears in her dreams or as whispering voices in her mind, and therefore the way her patron appears to her, is Lilly’s mind creating a mental representation as best it can based upon those experiences.
Her patron is a very ancient and exceedingly powerful Archfey. One that has existed since the Feywild first came into being, and might have even had a hand in its creation.

I'm guessing either a Archfey or a Great Old one, assuming this is using the ones from the Handbook.
