The Surgeon's scalpel is a relic made from metal forged in a heart of a patient, forming this scalpel kills the patient, causing the agony of the patient to take over the blade and increase the lethality of its user.
While attuned to the surgeon's scalpel, your precision with instruments increases, and you gain a +1 bonus to weapon attack and damage rolls. Whenever you land a weapon attack while attuned to this scalpel, you gain a stitch (maximum of 15).
1-5 stitches, you begin entering a surgical flow, your strikes begin to cause opponents to bleed out as you target veins and other vital parts. when you land a weapon attack or unarmed strike, a creature begins to bleed. at the start of the creature's next turn it takes 1d4 necrotic damage for every attack that lands
6-10 stitches: you are acting as if you are in the operating room; your reflexes increase, and you gain a bonus to your AC and dexterity saving throws equal to the number of stitches/3 rounded down. Additionally, you also gain the benefits of the 1-5 stitches.
11-15 stitches, The surgeon's instincts take you over. Whenever you deal damage with a weapon attack or unarmed strike, you deal extra damage equal to your proficiency bonus. Additionally, as an action, you can sacrifice all stitches and target one creature within 30 feet. a operating table is made under the creature and it gains the restrained and prone conditions until the end of your next turn. If it takes more than 25% of its damage before the end of your next turn, one of the creatures limbs are taken off. if the creature has no limbs or can't lose a limb for other reasons, it takes 10d10 necrotic damage. Additionally, you also gain the benefits of 6-10 stitches feature but not the 1-5 stitches feature
