Hard to tell if some of these spells were buffed or just not written correctly. Half of the spells I've looked at are missing key lines/notes or have terrible wording. Did they make it so there is no save to the 12d12 damage? or did they just forget to put that in? who knows!?
It didn't have a save for the psychic damage in the playtest, so no, that isn't an error. Also this spell doesn't have bad wording nor is it missing anything; it's pretty clear and straightforward. And it's a 9th level spell, so it should be that powerful.

Pretty nice buff since you can't polymorph characters into low hp forms to power word kill as easily anymore.

Nah, that's not true. 2024 Command is limited to options that are already provided, and even then, with the 2014 rules Command stated "the spell has no effect if the target is undead, if it doesn't understand your language, or if your command is directly harmful to it."
Does this new psychic damage go off if it's instant death effect is negated by Death Ward?
I wouldn't think so. The instant death effect and the psychic damage are mutually exclusive; one happens if you have more than 100 HP and the other if you have 100 HP or less. They can't both trigger on the same casting of the spell.
