Thousands of years ago, serpent people ruled parts of the world. Their empire centered on a continent they called Sshallossha with many cyclopean cities, its population devoted to the peaceful worship of Yig, the serpent god. Then this great race seemingly vanished overnight, their world-spanning civilization destroyed by the hand of the Unspeakable One, a loathsome deity born outside describable space. Most of the serpent people degenerated into savagery — but a few retained their sanity, including some of the cultists who had summoned the grotesque god. These apostates retreated to tunnels beneath Sshallossha, carrying on their uncouth rites beyond the wholesome glimmer of daylight and biding their time until the Unspeakable One could once again be persuaded to favor the world with its fearsome attentions.
While most of the followers of the Unspeakable One are ophidians and yuan-ti, they have recruited other races to their cult.

Each thrall is typically assigned the duties to which they are best capable of handling: grunt, sorcerer, priest, or warrior.

Delighting in furthering its own desires for chaos and power, the Unspeakable One has found that it can give tiny bits of that power to its loyal followers to hasten and strengthen its dominion. To that end, each true believer is gifted with some kind of power chosen from the list below:
- Stealth: Once per day, this follower may use invisibility on itself, as the spell.
- Swiftness: This follower's normal movement speed is increased by 10 ft.
- Vision: This follower's vision is enhanced, allowing him to see perfectly in all levels of light, including magical darkness, but suffering disadvantage in bright sunlight.
- Resilient: This follower has advantage on all checks vs poison and disease as well as resistance to one type of damage, chosen from one of the following during a 24 hour ritual -- slashing, bludgeoning, piercing, fire, cold, lightning, acid, thunder, necrotic, radiant, psychic, poison, or force.
