A caster with an axe to grind may have cast true polymorph for some petty offense committed by you or a loved one. Maybe you unwittingly donned a cursed magic item. Perhaps you activated some arcane trap when you accidentally stumbled into an ancient ruin. You could have made the choice willingly to hide from someone hunting you.

Due to your utter lack of resemblance to your real identity, you can't be recognized as who you once were. Though this can be helpful, it can be a hindrance. Most people you encounter will be unlikely to believe you if you try to explain your real identity, but the same goes for others attempting to reveal this information.
