You grew up in the Shadowfell after disappearing from your home plane as a child. Perhaps you were cruelly stolen from your old life by a strange black fog and abandoned in the Shadowfell. Perhaps you stumbled through a shadow crossing by chance while visiting a loved one's grave on some moonless night. Perhaps you were taken by the ever-hungering leagues of the dead but narrowly escaped with your life. Whatever the manner of your disappearance, you were forced to quickly adapt to survival in the Shadowfell and learned more than you ever dared to know of the dark denizens who live there.
When you finally returned to your home plane, you did not come back unchanged. You are haunted by the fact that the Shadowfell—a mirror world hidden behind a mere twist of perception—is only a hair’s breadth away. Try as you might to be rid of them, many of your memories of Shadowfell still haunt you and a creeping sense of paranoia and anxiety claw at your mind at the prospect of one day venturing back to the Plane of Shadows —your home away from home.

Whether the threat is real pursuer seeking to destroy you or it is only a figment of your troubled imagination which has manifested from feelings such as guilt or sadness, you will never truly know peace until you are rid of your dark pursuer. Roll on the table or work with your DM to determine what creature is pursuing you, then decide why this creature has a grudge against you and whether it's a manifestation of your mind or a real creature.

Those who look into your eyes can see that you have faced unimaginable horror and that you are no stranger to darkness.
