You died and a cleric had tried so many spells to bring you back then that you laid in your place of death completely unrotting for decades. The magic had finally worked though and has brought you back to life in the form of a new body. You can't remember everything about your past life, but you occasionally get visions of it in your dreams or when you are in meditation of some kind. This magic that courses through you doesn't make you undead but you aren't mortal either, so you are isolated from most big cities. For the fear and pain of your death still follow your new life in the form of a tattoo, reminding you of how you died.

Prior to being reborn, you lost all your memories except for the memory of the cold sting of death that claimed you. Now your flesh has been marked with a symbol of your death so that clerics may see that you have been reborn. Choose a deathly mark that brands you, or roll one on the Deathly Mark table.

There may be people in the world that still remember your past life and can identify you even if your form and personality have changed. Though they might detest you at first, they still have a bond with your prior self and would do anything to aid you. Unless you have shown yourself to be a danger to them, they will even take up arms to fight alongside you, should you find yourself facing an enemy alone.
