Whilst others might be in the business of ending lives, you do your best to save them. You might be a surgeon, physician, or medic, but your patients refer to you as a doctor all the same. Perhaps you studied in an academy or were taken on as an apprentice by an accomplished practitioner.
When creating your character, keep in mind what type of doctor they are and what kinds of values they have. Doctors generally take an oath to do no harm and to help patients to the best of their ability, but your character might have been self-taught, or trained by nefarious factions. Perhaps you are a selfish person who extorts money from the poor, or an assassin who uses their status as a doctor to get closer to targets.
Why did your character become a doctor? Perhaps they have close family or friends who died of illness. Perhaps they were drafted into a war while still trained. Due to the nature of their profession, doctors often see the best and worst of people. Consider how your character might deal with the death and pain they have witnessed. Do they distance themselves from everyone and only interact with them professionally? Or do they turn to religion for spiritual fulfilment?
