Incurable Image

You are assailed by a non-contagious affliction or curse so potent that medicine and miracle have failed you. Clerics lacking divine favor and doctors are confounded in your presence. Whatever this condition that gnaws at you cannot be slain with a sword or banished with a spell, and will one day bring you to your inevitable end. In desperation you might seek a more potent solution, strive to make your mark on history, or even accept your fate and press on hindered, yet unwavering in one last dutiful act. Living in life's extremes has brought you face to face with all sorts of people, namely priesthoods, those on the fringes of society and other outcasts seeking a place in the world. If there is one solace to this unenviable situation, it is that you're in good company.

Additionally, having confronted your mortality first hand, those who look into your eyes can feel your quiet determination simmering beneath the surface. Though some may fear your illness, people respect your resolve to live. You have became accustomed with various healers and priesthoods who extend to you shelter and medical comforts. You may also receive small discounts when purchasing alcohol or illicit substances to combat your pain.

Variant Incurable: In Sickness and in Health

This variant seeks to add additional rules and mechanics for dealing with a painful and life-threatening affliction over a long period of time. Consider using this if you want to add a sense of urgency to the goals you have set for your character. Furthermore when playing using this variant, consider the added difficulty of the mechanics and that your character may die before finishing their story in a satisfying manner. Discuss with your DM on the rarity of and what might be required to find a cure or if you want one to exist at all.

 
Variant Feature: At Your Limit

The Affliction you suffer from is severe and untreatable by regular means. While not contagious, it eats away at the body slowly and painfully until eventually you bodily functions fail and you perish. Standard resurrection is possible but does not function as a cure, and without one you maintain any penalties  that are a result of your Affliction through resurrection.

While suffering from your Affliction you have a affliction counter. At the end of short or long rest "At Your Limit" triggers and increments the affliction counter by one up to a max of three. After filling the counter you must succeed a DC 15 Constitution Saving Throw or suffer a permanent -10 penalty to your Max Hit Points. A creature within 5ft of you, proficient in the Medicine skill can choose to help you prevent a failure by succeeding a DC 13 Medicine Check. The affliction counter then resets back to zero.

If your max Hit Points hit zero while suffering from the affliction you die. This effect can not be removed by Greater Restoration and similar effects.
