You have seen many kinds of people, many kinds of mental illnesses, and many types of problems. You may not have been able to help everyone you met, but you certainly helped at least someone.
When you choose this background, you should think of how it was like to help the mentally ill and troubled, and how it might've affected your character. What kind of experiences did your character have? Why were they a Therapist in the first place? Were there any patients who deeply affected you? Why did you quit your job to go adventuring?

1 Your time working there was too traumatic, so you left.
2 A recurring patient with no record of mental illness told you about something wonderful, whether it be a great treasure, or a mystic land, so you set off to look for it.
3 One of the patients in your care absolutely destroyed your workplace, be it through a sort of terror act or by magic.
4 You run the hospital you work at, but it hasn't seen very good business, so you set off in hopes that you'l fund your hospital with money you find while adventuring.
You know how to talk to people. While in the company of people who you are not actively threatening with your actions, or who have no interest in talking, they are more inclined to be calmer around you, and are more likely to open up, and give you information you want to hear.
