"Some are haunted by voices in their heads. You have always known yours was real."
Whether by fate, forgotten magic, or a mystery lost to time, you have never walked through life alone. Two minds inhabit a single body, each with their own memories, ideals, fears, and ambitions. Though you share flesh, your thoughts are your own, and your recollections rarely align perfectly. Years of coexistence have taught both of you cooperation, compromise, and occasionally conflict.
To the outside world, you are one individual. To yourselves, you are two people sharing a single life.
- Ability Scores:
Your unusual existence has shaped your mind more than your body. Living alongside another consciousness has sharpened your perception, strengthened your understanding of others, and forced you to examine yourself more deeply than most ever will. Whether through study, survival, or endless internal conversation, you have learned to rely on insight as much as intellect.
(This is purely descriptive if you're using the 2024 Background rules where ability score increases are chosen separately.)
- Feat:
Your background does not grant a unique feat. If your campaign uses the 2024 rules, choose an Origin Feat as normal or work with your DM to select one that best reflects your upbringing.
(Or replace this with a custom feat if your DM wants one later.)
- Skill Proficiencies:
Choose Medicine and Insight.
One identity devoted itself to understanding the body, while both have spent a lifetime learning to recognize the subtle shifts of another mind—especially the one sharing their own.
- Tool Proficiencies:
Gain proficiency with Alchemist's Supplies.
Years spent preparing medicines, tinctures, restorative mixtures, and experimental compounds have made careful alchemical work second nature.
- Languages:
Learn one language of your choice.
Whether discovered through study, remembered from dreams, or inexplicably familiar to only one of your identities, this language hints that your shared past may be more complicated than either of you realizes.
- Equipment:
You begin with:
- A well-worn physician's journal filled with observations, sketches, and conversations between your two identities.
- A set of Alchemist's Supplies.
- A pair of spectacles.
- A set of common clothes.
- A belt pouch containing 15 gp.
The journal contains entries written in noticeably different handwriting, though neither identity remembers writing every page.
Use this table to help distinguish the identities sharing your body. Choose or roll for mannerisms, habits, or subtle differences that become apparent depending on which identity is currently in control.
| d6 | Fragments of Identity |
|---|---|
| 1 | One identity always removes their glasses before difficult |
| 2 | Your handwriting changes dramatically between identities. |
| 3 | One identity remembers names; the other remembers faces. |
| 4 | You unconsciously change your posture and gait when control shifts. |
| 5 | Each identity has a different favorite drink, food, or comfort ritual. |
| 6 | One identity occasionally recognizes people or places the other has never seen. |
The two identities sharing your body do not perfectly share memories.
At the Dungeon Master's discretion, one identity may recall forgotten details, recognize people or places the other does not, awaken from vivid dreams carrying unfamiliar memories, or instinctively understand information that cannot be readily explained. Journals, sketches, and personal notes may occasionally appear in handwriting, styles, or voices unfamiliar to the identity currently in control.
The Dungeon Master is encouraged to use this feature to introduce story hooks, reveal hidden truths, foreshadow future events, or create mysteries surrounding your shared existence. Likewise, the player is encouraged to embrace incomplete knowledge and roleplay the differences between their identities, even when doing so complicates their decisions.
This feature grants no direct mechanical benefit but serves as a collaborative storytelling tool between player and Dungeon Master.
Suggested Characteristics
Living with another soul has shaped every aspect of your life. Though you share a body, each identity has developed its own outlook, habits, and convictions. Some twin souls cooperate seamlessly, while others disagree on nearly every decision. Whether your relationship is one of friendship, rivalry, duty, or reluctant acceptance, both identities should feel like complete individuals rather than different moods of the same person.
When creating your character, consider choosing separate Personality Traits, Ideals, Bonds, and Flaws for each identity. Their differences can enrich roleplaying, create difficult choices, and provide opportunities for character growth throughout the campaign.
Neither identity needs to be wholly good or evil. They may simply view the world through different philosophies, each believing they are acting in the body's—and the other's—best interests.
