You've lived most of your life in a small village at the edge of civilization. Healing has been your life for as long as you care to remember. You might come from a line of doctors, or maybe the town's previous doctor saw something in you and took you under their wing. You may have been a medical student in a big city who, through circumstances no doubt extremely unjust, was forced to leave everything you knew to live in effective exile in the sticks.
You spend most of your time delivering babies, patching up minor bumps and bruises, dispensing medicines for minor illnesses, and comforting the dying. You might even pull double duty as the town vet, tending to sick and injured horses and cows. In times of crisis, you might have gone along with the town militia when it had to fend off bandits or orc raids.
The citizens of your town rely on you, and can't really pay you much, but appreciate you all the same.

Once people know you're a physician and healer, you'll be viewed generally positively in most communities, and you'll find it easy to find work pretty much anywhere you go. In conflicts, you'll often be viewed as a neutral party, since your loyalty is assumed to be on the side of life and safety.
