You died, and yet you live. A mortal resurrected you; Maybe a friend, maybe a powerful wizard, it could even be a cleric, but it wasn't a direct act of a god. The very laws of existance had to be ignored to bring you back, and as such the proccess couldn't have been perfect to begin with. Your mind, soul and sanity are not as they were, and it seems to be only a matter of time before they shatter forever.
Whenever a character is resurrected not through the direct action of a god, there's a 5% chance for each day since its death of this background being applied to it.
If the character is resurrected again, with a day (or more) passing since it died, this backrgound is automatically applied.
Note: this background is meant to give a resurrected character a limited time to live - the sanity checks are designed to spiral out of control quickly. If that's too much for your taste, feel free to adapt it.
Your grip on reality isn't is as firm as you would like, and it trying to shake you off isn't helping. You suffer from a Mental Condition, chosen from the following (or anything else the DM agrees to). This condition becomes debilitating whenever you fail a sanity check, and you lose control of your character for 2 (1d4-1) minutes.

As if creation itself rejects you being alive again, your life becomes insufferable with your sanity slowly but surely breaking under the pressure. Add the following stats to your character:
Insanity Score. starts at 0 and increases by 1 for each failed sanity check; resets when you reach a new Insanity Level. This score decreases by 3 after a long rest and by 1 after a short rest.
Insanity Level. starts at 1 and has the following progression:
- Level 1 to level 2 - requires an Insanity score of 5
- Level 2 to level 3 - requires an Insanity score of 10
- Level 3 to level 4 - requires an Insanity score of 15
- Level 4 to level 5 - requires an Insanity score of 20
Notes:
Each insanity level above the first adds a disadventage dice to your sanity checks.
Once at level 5, the player loses control over his character, as at this level it is completly insane.
Sanity Save Score. is equal to your highest abilty modifier minus your current sanity score.
Sanity Save DC. is always 10.
Suggested Characteristics
You are not an undead. You are you. The people who loved and cherished you before you died are nothing short of euphoric. You retain your bonds, your personality traits and your flaws. You were given a second chance to make things right and experience life to its fullest.
However, as the days since your resurrection passed, you began realizing that something went horribly wrong. Vivid nightmares rule your nights; you often hallucinate during the days. You start losing your sense of self and grip on reality. On top of that, the souls of the dead are trying to use you as a conduit to their loved ones; all you can hear are maddening whispers. The worst part is that the only whispers you can understand are the ones that ask you to let them in.

Note that this does not grant you any means to contact them other than those that you might already have (for example, if the deal was with a powerful fiend, you will still need to find a way to reach its plane in hell or find someone who can summon it).
