Something in your ancestry, or some kind of more recent curse, radiation, potion, poison, or blessing maybe, has resulted in an extra eye developing in your forehead. It looks almost the same as your other eyes, has all the necessary anatomy like eyelids, bone structure, optic nerve, to be a fully functional eye. It is also closer to your brain, pineal gland, ajna chakra, and other mystical seats of perception, and therefore perceives more of reality than your normal eyes. You gain Truesight for 60 feet, +3 to your perception and investigation skill checks, and +5 to your passive perception and investigation.
It is not subject to any problems that your other eyes suffer from like short- or long-sightedness, astigmatism, etc. However people sometimes find themselves unconsciously distracted by it when you're talking with them. Whenever you roll for persuasion or deception, you suffer a -3 penalty to your total score. If this causes you to fail a roll, the DM decides whether someone was rudely gawking at your third eye instead of listening to you properly (or possibly idly pondering it, fascinatedly studying it, wonderingly marvelling at it, jealously coveting it, etc)
