Lycanthropy

You are now cursed with lycanthropy of the wolf, details of the curse are here:

Ability Score Increase

Your Strength score increases by 2, and your Strength score maximum increases by 1 (Maximum 30)

Keen Hearing and Smell

You have advantage on Wisdom (Perception) checks that rely on hearing and smell

Wolves Hide

Your Wild Animal forms bear a thickened hide and enhanced reflexes making you hard to hit. While In your Wolf or Hybrid form, your AC is Equal to 10 + Dexterity Modifier + Your Constitution Modifier.

Shapechanger

You can use your action to transform into a Wolf-Humanoid Hybrid or into a Wolf, or back into your true form which is humanoid. If you fall unconscious or die you will revert back into your true form. Your Stats other than your AC are the same in each form. While in either wolf or Hybrid form your walking speed increases by 10 feet. Any equipment you are wearing or carrying isn't transformed, all nonmagical armour or clothing if not taken off prior may be either broken or destroyed! You can still use weapons, Class features and spells while in Hybrid From. You can not speak while in Wolf form. 

Pure-bloods or humanoids that have embraced the curse can transform at will, while those that resist the curse only transform under the light of a full moon or magical means. The transformation lasts for a number of hours equal to your Constitution modifier, after returning back to true form, you receive 1 point of Exhaustion.

Tooth and Claw

While in Hybrid or Wolf form you can use your attack action to use your Claws or Bite attack. Your Bite carries the Lycanthropy curse If you bite a non-lycan Humanoid they must make a Constitution Saving Throw equal to 8 + Your Proficiency Modifier + Your Constitution Modifier, on a failed save they are Cursed with Lycanthropy. On a success, they aren't affected and are Immune to the curse for 24 hours.

Body of the Beast

The Lycanthropy curse effects the body giving Immunity to disease and slows down ageing by half once reaching adulthood. Also while in Hybrid or wolf form you are Immune to Bludgeoning, piercing and slashing nonmagical attacks that aren't silvered or laced with wolfbane.

You are however now vulnerable to Silver and Wolfbane, which inflicts serious harm and discomfort to you.

Regeneration

You regain Hitpoints equal to your Constitution Modifier at the start of the turn while in Hybrid or Wolf form and not in contact with either silver or wolfsbane. If you take damage from an attack with silver or wolfsbane, this trait doesn't function at the start of your next turn. 

Full Moons Curse 

Lycanthropy is an ancient curse, which under the light of the full moon the beast within can not be contained and the transformation is forced upon the afflicted. At the moment the Moons rays glance the skin your transformation begins and wild thoughts of the hunt cloud your mind. You must make a DC 13 Wisdom Saving throw,  succeed you transform and remain in control, fail you lose control and run wild and the DM takes control until you revert back or the moon wanes. You remember nothing but horrific dreams of the wild nights.
