Being infected with the curse of lycanthropy, you can use your action to polymorph into a beast-humanoid hybrid or into a beast, or back into your humanoid form. You automatically revert to your humanoid form after 1 hour, or if you fall unconscious or die. The type of beast you transform into is determined by the lycanthrope that infected you.
You are vulnerable to piercing and slashing damage from silvered blades. While transformed, however, you gain a +1 bonus to AC, resistance to bludgeoning, piercing, and slashing damage from nonmagical attacks that aren't silvered, and can make Bite or Claws attacks.
After you hit a non-lycanthrope humanoid with a Bite attack, it must make a Constitution saving throw against a DC equal to 8 + your proficiency bonus + your Constitution modifier, or be cursed with lycanthropy.
Additionally, your senses are now permanently sharpened - you have advantage to Wisdom (Perception) checks that rely on hearing or smell.
While you can assume your beast form at will, it can also be forced upon you by certain circumstance.
Call of the Moon. Whenever two or more moons are full, you feel the call of your wild side. You must make a DC 12 Wisdom saving throw as soon as you're touched by the light reflected by one of the full moons or immediately use your action to transform into either the hybrid or the beast form. You remain in that form until sunrise and cannot willingly change it until then. On a successful save you are immune to this effect for 24 hours.
Urge for Blood. If you drop below half of your hit point maximum while in your humanoid form, the urge of the curse begins to overtake you. You must make a DC 12 Wisdom saving throw at the start of each of your subsequent turns, or immediately use your action to transform into the hybrid form. You can choose to fail the save and give in to the forced transformation.
While transformed in this way, you must continue to make the saving throws. On a failure, you must attack the creature nearest to you, regardless if it's friendly to you. If there is more than one creature within the same range, you can choose which one to attack.
The urge stops when you drop to 0 hit points, regain enough hit points to negate the initial triggering condition, or when there are no conscious targets you can see or smell within 300 ft of you. You remain in the hybrid form as long as the urge persists.
Once the urge ends, you gain a level of exhaustion, and you are immune to the urge for 1d4+3 days.
