You have befallen an ancient curse, one where man becomes beast in the light of the moon. As an action, you may shift between your human, hybrid, and animal forms, depending upon your specific curse. All of your equipment and armor is absorbed into your form when you shift. While in your animal or hybrid forms, you gain resistance to non-silvered bludgeoning, piercing, and slashing attacks; you also gain additional attacks depending upon your specific curse, as well as darkvision with a range of 60 feet.
You stay in this form either for 1 hour or until you use your action to turn back. During a full moon, the transformation is compulsory and lasts all night, and you cannot voluntarily change back until the night is over.
Whenever you are hit by an attack, you must make a Wisdom save with a DC equal to the damage dealt. On a failure, you go into a ferocious rage, and must use your next action to attack the nearest creature to you.
Your stats, including ability scores and armor class, while in animal or hybrid form remain the same unless otherwise stated. Whenever you make a bite attack against a living creature while in your animal or hybrid forms, the creature must succeed on a Constitution saving throw equal to 8 + your proficiency bonus + your Constitution bonus or develop Lycanthropy as well.
