Stage 1: Early Infection

  • Damage: One point of Constitution and one points of Dexterity are lost. Their maximum hit point level goes down by the maximum of their hit die for their class. They also take one level of exhaustion that cannot be corrected through a long rest.
  • Indications: The earliest signs of ghoul fever include pallor, mild fever, and an inexplicable fatigue. Victims may also experience discomfort in bright light and a gradual loss of appetite for normal food, developing a craving for raw meats.
  • DC to Notice: A DC20 Medicine check is required to discern these early signs from a common illness. Observers might notice the victim's unusual sensitivity to light and changes in dietary preferences.
  • DC to Resist Advancement: A DC12 Constitution saving throw is required at the end of each day following the initial infection. Success means the victim's condition does not worsen for that day.
  • DC to Overcome Disease: A DC14 Constitution saving throw is required to fully recover from the infection at this early stage without the need for magical intervention. This save can be attempted once per day.
