Stage 2: Advanced Infection

  • Damage: Three points of Constitution and Three points of Dexterity are lost. Their maximum hit point level goes down by the three times the maximum of their hit dies for their class. They also take two levels of exhaustion that cannot be corrected through a long rest.
  • Indications: As the infection progresses, the symptoms become more pronounced. The victim's eyes begin to sink, and their skin turns a sickly, pale hue. Minor aggression and mood swings are common, with an increased craving for raw flesh. At this stage, faint necrotic spots appear on the body, and the victim's nails and teeth may start to elongate unnaturally.
  • DC to Notice: A DC15 Medicine check is needed to identify these symptoms as being related to ghoul fever. The aggression, coupled with physical changes, provides a clearer indication of the infection's supernatural nature.
  • DC to Resist Advancement: The DC increases to 15 for daily Constitution saving throws to prevent the disease from advancing to the final stage.
  • DC to Overcome Disease: Overcoming the disease at this stage is more challenging, requiring a DC17 Constitution saving throw. As with stage 1, this save can be attempted once per day, reflecting the victim's natural attempt to fight off the infection.
