Wild Form Image
Prerequisite: you must be a Wild Magic Sorcerer or Wild Magic Barbarian
 

Your Wild Magic has taken a slightly different form over the course of your adventures, instead of a random effect occurring when you roll Wild Magic, you turn into a random creature, listed below, you use their statblock until the form ends. Also, when you roll wild magic, roll 1d10, on a 1, you lose consciousness and behave like that creature would until the form ends, the form ends after 1d4 hours or until you drop to 0 hit points

1- You become a Rabbit

2- You become a Beholder

3- You become a Skeleton

4- You become a Owlbear

5- You become another random player race (DM's choice), don't roll a d10

6- You become a Mindflayer

7- You become a Turtle

8- You become a Wolf

9- You become a Aboleth, if you aren't in water, reroll this, if you roll this again, you become a Aboleth regardless if your on land or not

10- You become a Young Red Dragon, if you are a dragonborn, become whatever color Dragon your Dragonborn is

11- You become an Imp

12- You become Animated Armor, your AC is equal to whatever armor you were wearing before being transformed

13- You become a Vampire Spawn

14- You become a Vampire Lord

15- You become a Cuatl

16- You become a Kobold

17- You become a Goblin

18- You become a Mimic

19- You become a Barbed Devil

20- You become a Hill Giant

21- You become a Gelatinous Cube

22- You become a Hag

23- You become a Cambion

24- You become a Treant

25- You become a Awakened Shrub

26- You become a Potted Plant

27- You become a Chicken

28- You become an ABYSSAL Chicken

29- You become a Cyclops

30- You become a Fire Giant

31- You become a Bandit, your Race is random (DM's choice)

32- You become a Acolyte, your Race is random (DM's choice)

33- You become a Cultist, your Race is random (DM's choice)

34- You become a Fairy

35- You become a Homebrew monster of your DM's choice, or they can make you reroll this table

36- You become an Elemental of whatever damage you did last, if the element you did damage with doesn't have a elemental, you can pick

37-  You become a Giant Badger

38- You become a Bat

39- You become a Griffon

40- You become a Boar

41- You become a Deep Rothe

42-  You become a Tarrasque, this transformation only last for 12 seconds, (2 rounds) and you automatically roll a 1 to determine if you still have your mind while in this form

43- You become an Ancient Dragon of your choice

44- You become a Crab

45-  You become a Abominable Yeti

46- You become Infant Basilisk

47- You become a Bulette

48- You become a Myconid

49-  You become a Banshee

50- You feel overwhelming power coming from you, you may choose any form on this list to take, the duration is halved.
