Prerequisite: Getting your heart thoroughly mutilated but not destroyed.
There's only so much normal healing magic can do to repair severe damage, your heart has been damaged so severely that it is significantly weaker than it was before. Your constitution is lowered by 4, and you have disadvantage at CON saves. Only a regenerate spell or a replacement (mechanical or magical) can fix your broken heart.
