Candle Burning at Both Ends
Adventurers often disregard grievous injury to keep fighting, sacrificing their future well-being for immediate survival. You gain the following benefits:

Increase your Constitution score by 2, to a maximum of 20.
Relentless Survival: When you would fall to 0 hit points or die, you can choose to endure exhaustion or permanently mark a death saving throw to return to the fight with renewed strength. This ability will not activate if a character were to die to old age.

You can choose one of the following effects:

+2 Exhaustion: You restore (2d8) + (Constitution modifier) hit points.

+3 Exhaustion: You restore (6d8) + (Constitution modifier) hit points and regain one expended spell slot of level 7 or lower.

+4 Exhaustion: You restore (12d8) + (Constitution modifier) hit points and regain two expended spell slots of level 7 or lower.

+1 Permanently Marked Death Saving Throw: You restore all hit points, regain all expended spell slots, class abilities, and hit dice, and immediately stand up. An aura of protection radiates from you, granting a +5 bonus to your AC for 10 rounds.

Long Term Consequences of Permanently marking a Death Save:

12-hours after using this ability, make a Constitution saving throw with disadvantage. On a failure, your Constitution score is permanently reduced by 3. On a success, roll a d4: on a roll of 1-2 your Constitution is permanently reduced by 2; on an 3-4 it is reduced by 1.

Each time you permanently mark a death save, the number of failed death saves required to kill you decreases by 1 (e.g., from 3 to 2).
The number of successful death saves required to stabilize increases by 1 (e.g., from 3 to 4).
You can use this ability up to 3 times.
If you fall to 0 hit points or die with 3 permanently marked death saves, you and everything you are carrying turn to dust.

Ways to remove permanently marked death saves may exist in your world through acceptable ways to remove these marks are ultimately up to your DM. For example taking a significant amount of uninterrupted downtime (1-2years), from greater beings/entities, or through high magical means.
