Be it from an old wound, a vengeful spirit, a curse, years of isolation, or as a manifestation of hunger, a second mouth appears either on the back of your head or in a large matting of hair.
The second mouth provides many benefits and disadvantages alike:
- Increased Awareness: All surprise attacks against you are made at a disadvantage.
- Unnatural Physiology: Those unaware of your condition can often sense an unnerving sensation around you, making them more likely to be suspicious of you. When meeting a person or entity for the first time, make CHA saving throw. On a failed throw, the person or creature you are exposed to takes 2d4 Psychic damage and becomes Frightened.
- It Speaks: At times, the second mouth whispers to you in the dark. Once a day make a WIS saving throw. On a failed save take 2d4 Psychic Damage, and on a successful save take none.
- I Have Two Mouths, And I Must Scream: All breath and shout based attacks have advantage. Additionally, such actions can be made directionally backwards or forwards; or (in exchange for an action) made AOE, with a radius of the original spells range.
- Reactive Bite: When attacked from behind within melee range, take a reaction to bite your opponent and deal 1d4 damage.
- Letting My Hair Loose: As a Bonus Action, let loose a tendril of hair (assuming you have any) like a whip, striking one target within a 5 foot range with 1d8 Bludgeoning Damage. This target then makes a 15 STR saving throw, or becomes Grappled and moved 5 feet to either the left or right, or closer to you.
- Extra Limb: If you a STR of 16 or higher, you may use one of your tendrils of hair to hold 1 weapon with the Light or Finesse Property, and make 1 extra attack per round.
