You have inherited the ancient and forbidden blood of a vampire. Whether born of a dark pact, a cursed lineage, or the bite of a forgotten Coven, the thirst now lives in you. You keep this secret hidden—only the most trusted know the truth. With this gift comes great power, but also grave consequence.
Vampiric Curse: Feeding and the Thirst
You must feed on the life essence of a living creature (beast, humanoid, monstrosity, etc.) at least once every 3 days. This feeding does not need to kill the creature, but it must be a deliberate act of consuming blood or essence.

Feeding Schedule:

3 Days Without Feeding: You gain the Weakened condition — you have disadvantage on all ability checks and attack rolls.

5 Days Without Feeding: In addition to being Weakened, your movement speed is halved.

7 Days Without Feeding: Your body withers and dies. You are permanently dead, unless revived by magic that restores the soul.

Important Note: Feeding on a creature resets the timer to Day 0.
Sleep and Hunger
You do not suffer from exhaustion. However, each day you choose not to take a long rest, it counts as an additional day of not feeding. This is your "exhaustion."
Example: If you have not fed in 1 day and choose to skip your long rest, you are now considered 2 days without feeding. If you feed the next day, the count resets.
The Bite That Spreads (Level 15+)
Once you reach 15th level, your blood has matured enough to pass on vampirism—though it is rare, forbidden, and dangerous.

Once every 30 days, you may choose to infect a creature through your Blood Drain.

The creature must succeed a DC 15 Constitution saving throw, or they begin transformation into a vampire spawn under DM discretion.

This process may have narrative and mechanical consequences, and is highly discouraged by most cultures and guilds. Few will forgive you for turning another.
