You gain a sixth sense and can tell when the party is on their last legs with no way out. So you become the way. When you are at 25 percent of your health or lower you can use a reaction to teleport all party members including deceased with or without a physical body and within a 30ft foot radius to the last safe location you remember whether it be the tavern, a clearing you passed etc. When all in your party arrive at the location ,all who are making death saving throws are stabilized. You do not teleport with them. You can only hope your sacrifice will count for something.
