Insanity Scale: Barovia’S Grip Image

In Barovia, madness isn’t a condition it’s a curse. Every horror chips away at your mind. The Mists don't just steal your body. They crave your soul.

Level 0 – Clear Headed
You’ve seen strange things, but you still know what’s real. You still believe in escape. No penalties. Full control of thoughts and emotions. Can recover easily with safe rest or holy sanctuary.

 Level 1 – Uneasy Whispers
You flinch at sudden sounds. The quiet seems louder now. When you're alone, you hear breathing that isn't yours. RP effect: Mild paranoia or anxiety. Mechanical: None, but narrative tension builds. Can be cleared with rest in safe, comforting conditions.

Level 2 – Paranoia You question others' motives even friends. Shadows seem longer. Something watches you. Disadvantage on Insight and Persuasion checks involving trust. You begin suspecting your party or hallucinating minor sounds/movements. A short rest in a blessed or peaceful location may reduce this.

Level 3 – Distorted Perception
Walls move. Faces change. You hear someone calling your name, softly… sweetly. Disadvantage on Perception and Investigation checks. Once per session, you must succeed a DC 13 Wisdom save or take 1d4 damage A long rest in a blessed or peaceful location may reduce this.

Level 4 – Delusional
You're certain: you're cursed. Someone replaced your blood. Strahd speaks through the wind. You develop a persistent delusion (e.g., “I'm already dead,” “My party serves Strahd,” “Only fire is safe”). Wisdom save (DC 15) required to act against your belief. (-1) to all saving throws. Requires Greater Restoration, exorcism, or a major roleplayed turning point to recover.

Level 5 – Shattered Mind
You laugh, weep, or scream for no reason. Words no longer hold shape. The Mists speak your name. (-2) to all Wisdom and Charisma checks. At dawn, roll a DC 17 Wisdom save or suffer a short-term madness Every nightmare is real to you. Others seem like ghosts, echoes. Can only be treated with Greater Restoration and prolonged spiritual recovery.

Level 6 – Broken (Final State)
Barovia has claimed your soul. Upon reaching this level: The character dies (mentally overwhelmed) or becomes Strahd’s thrall a puppet, servant, or even a dark echo of themselves. DM’s Choice: They may be turned into a Named NPC Villain, vampire spawn, undead knight, or possessed body used against the party. Player loses control of the character. A rare, complex ritual might reverse this fate but not without cost.

    Insanity Check
When exposed to traumatic, horrific, or reality-bending experiences, the DM may call for an Insanity Check to resist slipping deeper into madness.
     Check Type: Wisdom Saving Throw
DC: Typically 10–25, based on severity of the event. On a failure, the player gains 1 Insanity Point (or advances one level on the Insanity Scale)., On a success, they steel their mind but repeated exposure may still take its toll later.
