When you are reduced to 0 Hit points, roll a d4. Apply the rolled effect.
1: Psychic Screams: Your mind releases the voices of those lost in the void, casualties of experiments gone awry. Each creature in a 20-foot-radius sphere centered on you makes a Charisma saving throw, taking 8d6 Psychic damage on a failed save or half as much damage on a successful one.
2: Rise, My Child: You are drawn back to consciousness. You regain d6 Hit Points, and you take d4 levels of exhaustion. You can only roll this result once per Long Rest.
3: The Void Hungers: The tendrils of a long forgotten entity lash out from your body. A 20-foot-radius Sphere of Darkness appears, centered on you and lasts 1 minute or until you gain 1 or more Hit Points. The Sphere is Difficult Terrain, and it is filled with strange whispers and slurping noises, which can be heard up to 30 feet away. No light, magical or otherwise, can illuminate the area, and creatures fully within it have the Blinded Condition. Any creature that starts its turn in the area takes 2d6 Psychic damage. Any creature that ends its turn there must succeed on a Dexterity saving throw or take 2d6 Force damage from the otherworldly tendrils.
4: Silencing Aura: Your thoughts are filled with visions of your allies suffering harm that is outside of your control, and your very being opens a section of the Void to silence it. An aura of antimagic surrounds you in 20-foot Emanation. No one can cast spells, take Magic actions, or create other magical effects inside the aura, and those things can't target or otherwise affect anything inside it. Magical properties of magic items don't work inside the aura or on anything inside of it. Areas of effect created by spells or other magic can't extend into the aura, and no one can teleport into or out of it or use planar travel there. Portals close temporarily while in the area.
