You made a pact with the Fey, discovered the changeling, or perhaps traded a humanoid child of your own in exchange. However you came upon the child, it now is bound to you and you to it. It has taken the form of a humanoid infant - a form it will keep as long as it wills it. To all but the most careful observer, you have a beautiful child. To the astute observer, something is terribly wrong.
You gain the service of a Fey Changeling, whose full stats are described on the Fey Changeling monster sheet. Additionally, you gain the Summon Fey Changeling spell, which does not count against your spells known.
