Once per round when a creature within 5 feet of you transforms, reveals its true form, or stops disguising itself (such as a shapechanger or magically disguised creature), you can use your reaction to make one melee weapon attack against that creature. If the attack hits, it deals an additional +1d6 radiant damage.
