The Chirurgeon almost seems benevolent - albeit horribly misguided. His skills, both supportive and offensive, all seem to be performed with the aim of treating whatever maladies he perceives his companions to have. Indeed, these treatments occasionally prove fruitful, with Leucotomy both healing and buffing his party "patients,"
The Ward Within Scenario Trait You know what you are carrying. A shard of the same sick radiance that warps this city sleeps inside whatever is left of you, and your first duty, before the mission, before your patients, before anything, is containment.
Near the void-touched surface of Marrowdeep, the passenger is quiet, almost soothed, and you are at your sharpest: you have advantage on one chosen ability's checks while in void-touched surface regions.
The deeper you descend toward Underdeep, the more it strains against you: your checks and attacks suffer increasing impairment the further down you go, at the DM's discretion.
One more thing. Something hunts you in the dark hours. It is patient, it is gentle-voiced, and it wants to be found. Do not find it. The twisted places of the void offer you a strange shelter from it. Use them.
Night Surgeon Archetype: The Chirurgeon While the others rest, you work. You have 2 Surgery tokens each night. Each token performs one procedure on one patient (you may operate on yourself). Each milestone of your personal quest permanently increases your nightly tokens by 1.
Procedure: Leucotomy Night Surgeon Option You restore what the city has taken. The patient's hit points are restored to their maximum as it stood the previous day, clawing back what the long rest ratchet has claimed. The surgery is real and so is the toll: the patient suffers the effects of the Bane spell until their next long rest. The hands remember the knife.
Procedure: Trephination Night Surgeon Option You open the skull and quiet what screams inside. Make a Medicine check, DC 20. All negative conditions and modifiers on the patient are masked for one day: Motivation penalties, void sickness, Bane, all of it. The patient feels cured. The patient is not cured.
The debt is banked, not forgiven. On a failed check, bank 1d6 Sanity damage per condition masked. On a success, bank 1d4 per condition instead. The debt grows each time you mask again. If the masking ever lapses, if a night passes without the procedure, the full banked debt strikes the patient at once as every condition returns.
Procedure: Lobotomy Night Surgeon Option The permanent cure. The terrible one. Only a patient suffering 3 or more levels of Motivation loss may receive it. One level of Motivation loss is healed, permanently, the only such cure in this city that does not require a quest fulfilled.
The price: the patient's maximum hit points are permanently reduced by 5, and they make Wisdom and Intelligence saving throws at disadvantage for one day while the mind knits around the wound.
You do not walk away clean either. After performing this procedure, you are Frightened, and the thing you are frightened of is yourself.
