Prerequisite: The ability to cast 9th level spells or have an 18th level spell caster assist in the completion of the ritual.

There are certain benefits this form allows me. I knew the cost would be great, but to me, it was worth it to end the torment. That's why I started collecting the things I needed for the ritual before I... died. I don't need your approval, nor do I seek it. I've justified this course of action long ago. My only regret is that I did not anticipate the reactions from my closest colleagues. I would never hurt them, but all I've received from them is threats and hatred.

Through some means you've learned the secrets to becoming a Lich, and have completed the necessary ritual. This arduous process has transformed you and made you into a powerful undead. You gain the following benefits:

  • Undead Nature - you do not require air, food, drink, or sleep.
  • You are resistant to Cold and Lightning damage.
  • Magic resistance - you have advantage on any saving throw against spells and magical effects
  • You are immune to the Charmed, Exhaustion, Frightened, Paralyzed and Poisoned conditions.
  • You are immune to poison, necrotic damage and Bludgeoning, Piercing and Slashing damage from non-magical attacks.
  • You gain the Paralyzing Touch action. Make a melee spell attack that deals 3d6 cold damage to a target within range. If hit the target must succeed a Constitution saving throw equal to your spell save DC or be paralyzed for 1 minute. The target can repeat the saving throw at the end of each of its turns, ending the effect on itself on a success.
  • The character's AC becomes 14 + the character's Dexterity modiier when not wearing armor.
  • Grant Legendary Resistance (1/long rest)
  • Truesight out to a range of 120ft.
  • You can Fear without expending a spell slot

You gain an ability Disrupt Life which you can use 3 times and you gain all expended charges of this ability when you take a long rest.

Disrupt Life. You can use an action and each non-undead creature within 20 feet of the lich must make a DC 18 Constitution saving throw against this magic, taking 21 (6d6) necrotic damage on a failed save, or half as much damage on a successful one.

Limited Immortality.  When you die your mind and soul are drained from your corpse as it falls to dust. In 1d10 days your body reconstitutes next to your phylactery. Coalescing from glowing smoke that issues from the device. However, the phylactery requires soul sacrifices to maintain a working order. You must periodically feed souls to your phylactery to sustain the magic preserving your body and consciousness.

A lich that fails or forgets to maintain its body with sacrificed souls begins to physically fall apart, and might eventually become a demilich.
