If your character has a sibling of some sort, and you two are close, then you gain this: You can feel what is happening to them. You can hear/talk to them with thoughts. You hear it as if they are talking to you. It seems pretty simple. No one else knows about your special bond unless that character tells the other person that they have a special bond with a certain person.
