Breath Of Horror Image
Prerequisite: Requires Wisdom 20 Requires Chaotic Evil

Starting at level 5 you feel the weight of your sins, the weight of all your pain crash down on your shoulders and fester in your mind. You suddenly gain an insatiable appetite for blood. The want to feast on another creature overwhelms you like an infection to the brain. This change splits your soul in half, one side containing the goodness of your heart whilst the other conceals its darkness, waiting to be released.

Once, and only once, when your HP reaches 0, you burst into a bloodlust fuelled fury, your malicious intent pulsating out of your face in the form of an ethereal demon. Any creature within a 15ft cone of your face must make a DC 10 + your wisdom modifier intelligence saving throw or be afraid of everything in the room, knocking them prone. Any creature that fails this check will also take 3d6 necrotic damage as your evil will continues its rampage into their very souls.

At level 13, your HP must only drop to 15 or below to activate Breath of Horror. Additionally, creatures who fail their saving throw take an extra 1d4 necrotic damage during their turn, your inner malevolence crushing their being further.

At level 20, your soul amalgamated back into its original state, you have conquered your bloodlust and hunger for flesh. Once per turn, using a bonus action, you can cover your face with your true evil and screech out. All creatures must make a DC 17 Wisdom saving throw or be feared by your very presence. This ability can only be used three times, then the user must take a short rest to use this ability once more, or take a long rest to regain all uses. The HP required to activate the primary ability of Breath of Horrors becomes 20 or below.
