You offered a limb to your patron and shall be rewarded. The severed limb is replaced by a new one that changes.
You learn the spell alter self to change your appearance without using a spell slot, originating from your lost limb.
Once used, you'll regain this ability after a long rest. Charisma is your spellcasting ability for this spell.
As everything comes with a price, there is a chance that the gift will change you involuntarily. This might happen for various reasons, for example, a sudden strong connection to your patron, a memory, or damage taken in battle. When it happens make a DC 15 Charisma saving throw, on a fail you'll transform even if the spell was already used.
The change will reflect your patrons appearance to a certain degree and some visual cues might linger after you transformed back (no mechanical benefits).
