You have succumbed to the disease "Hemostasis", passed your CON saving throws, and have been reborn as a Vampire.
Undead. You retain all your racial/class/background features and abilities, but your typing is now Undead. You do not require food or drink, you benefit from a short and long rest as normal.
Ablitiy Score Improvement. Your STR, DEX, and CON become 18 if they aren't already 18 or higher.
Traits. You gain the following traits.
Darkvision of 120'
Resistance to Necrotic damage; Bludgeoning, Piercing, and Slashing damage from Nonmagical Attacks that aren't silvered.
Proficiency in Perception and stealth if not proficient already.
Vampire Weaknesses. You have the following flaws:
Forbiddance - You can’t enter a residence without an invitation from one of the occupants.
Harmed by Running Water - You take 20 acid damage if you end your turn in running water.
Stake to the Heart - If a piercing weapon made of wood is driven into your heart while you are resting, you are paralyzed until the stake is removed.
Sunlight Hypersensitivity - You take 20 radiant damage when you move into or start your turn in sunlight. While in sunlight, you have disadvantage on attack rolls and ability checks.
Actions. You gain the following actions.
Bite: Your bite attack can be used on a grappled, incapacitated, or willing creature. Your bite is an unarmed strike that is 1d8+prof bonus+STR modifier of piercing damage, that also deals 1d6+STR modifier+prof bonus necrotic damage. The target’s hit point maximum is reduced by an amount equal to the necrotic damage taken, and you regain hit points equal to that amount. The reduction lasts until the target finishes a long rest. The target dies if this effect reduces its hit point maximum to 0. The target also needs to pass a DC 10 CON saving throw or be infected with 'Hemostasis' disease (see my homebrew feat 'Hemostasis Disease' for effects).
Charm: You can cast the 'Charm Person' spell a number of times equal to your CHA modifier per long rest. The DC for this spell is 8 + your prof. bonus + CHA modifier (you may need to edit this on the character sheet). Any creature charmed in this way is a willing target for your Bite attack.
Hunger for Blood. You feel the need to consume humanoid blood. If you fail to consume the blood of a humanoid once every 3 days you gain one level of temporary exhaustion that can only be removed by drinking the blood of a humanoid, which would remove all levels of exhaustion. You can consume blood in one of the following ways.
Blood Potion: Use an action to consume one Blood Potion (see my homebrew magic item 'Blood Potion' for effects).
Bite: Use your Bite action on a creature.
Recently Dead: You can use an action to drink the blood from a creature that has been dead for no longer than three minutes.
