Familiar Form Image
Prerequisite: Wizard, warlock or sorcerer with knowledge of the spell Polymorph.

Wizards, warlocks and sorcerers with knowledge of the "Polymorph" spell gain the ability to have a "Familiar Form" - one common animal shape they can change into once per short or long rest. They also retain their own Intelligence and Wisdom scores while in this single form. Once in their Familiar Form, the caster may remain in this form indefinitely, however they have no abilities other than those that their chosen animal form has naturally. Changing into or out of the Familiar Form is an action.

The shape that can be taken is of one specific creature only, and it must be a common form of animal - exotic animals (e.g. dinosaurs, enhanced ability animals, etc.) cannot be chosen as the form. While the DM will have discretion based on the types of animals that are common in the regions known to the character, types of Familiar Forms may include:

Wolf, bear, owl, fox, eagle, hawk, raven, bat, rat, weasel, squirrel, badger, deer, cat, dog, horse, boar, lion, tiger, panther, cougar, lynx, ape, orangutan, monkey, jackal, hyena, crocodile, alligator, rhinoceros, wild ox, seal, dolphin, otter, beaver, kangaroo, wallaby, wombat, possum, bat, goat, moose, albatross, seagull, etc.

The chosen form may be visually distinct from other animals of this same type through distinct patterns or coloration, but is in all other ways physically equivalent to animals of the chosen type.

The single shape chosen for this feat must be of a creature the character is familiar with in some way (for example, having previously interacted with animals of this kind, or having adopted this form using polymorph in the past). Once the Familiar Form is chosen, it cannot be changed. If knowledge of the spell Polymorph is lost, this feat becomes inactive.
