You have been pushed to the point at which your body and or mind breaks many times, but now you can do so to your advantage.
Many different things can happen when you use this feat: All physical stats double, you use a spell you don't yet have access to, don't use a spell but just summon some raw power, or you come back from unconsciousness, etc.(work with your DM on what's possible here)
But in exchange, you must experience a permanent injury(ex. destroyed appendage or extremity, can't do a certain type of magic, has a restricting and painful scar[work with your DM on this part too])
While this might get you out of extreme danger, the price is just as extreme
