Brain Blight Image

You have been infected with Brain Blight. The effects of this disease are progressive, and the effects of each stage of the disease stack with each other. 

Once per long rest, make a CON 13 saving throw. If you fail this saving throw, your Brain Blight infection progresses by one stage.  If you succeed, nothing happens. For every successful long rest under the effects of Brain Blight, the CON saving throw increases by +1.

Stage 1:

  • You have disadvantage on all wisdom saving throws and ability checks. 
  • Once per turn, roll a CON 15 + (stage number) saving throw. If you fail this saving throw, you must cough as an other action at some point during your turn. If you succeed, you resist the urge to cough. You take 1d4 damage (multiplied by the stage number of your disease) for every cough.

Stage 2:

  • As the symptoms of your infection become more noticeable, people want to stay away from you. You have disadvantage on all charisma saving throws and ability checks.
  • For every round spent in fighting in combat, make a WIS 13 + (stage number) saving throw; if you fail this roll, you gain a level of exhaustion. If you succeed, you resist and are not exhausted. 
  • You have disadvantage on all saving throws made whilst trying to navigate difficult terrain. 

Stage 3:

  • If you fail the CON save and cough on your turn, you waste your whole turn in a coughing fit. Under the effects of a coughing fit, all creatures have advantage on to-hit rolls made against you; in addition, the effects of a cough now apply to a 5ft radius around you.
  • A creature with Brain Blight must make a WIS 18 saving throw as a reaction after being reduced to 0 HP. If they succeed the saving throw, nothing happens and the creature dies as normal. If they fail the saving throw, the creature's head will explode, dealing 2d4 force damage to every other creature in a 5ft radius. 

Stage 4:

  • You automatically fail any charisma/wisdom saving throw or ability check. Attempting a charisma check on anyone will make them either run away or become hostile towards you. 
  • For every round spent fighting in combat, you automatically gain 2 levels of exhaustion without any roll needing to be made.
  • You have disadvantage on all saving throws.
