It’s not adding the extra +4 Strength from the weapon on top of the strength bestowed by the belt.

This definitely used to work properly, as I recall testing it before. I'm sure that it'll be fixed pretty quickly. :)
It's also worth noting here that the requirement for attuning to the hammer is to be wearing the belt and gauntlets - they don't have to be attuned.
