I am confused what the issue is with the medium armor feat. Medium armor only lets you get a bonus of +2 from Dex max per the rules. If you have 20 dex you still only get a +2. The feat allows you to have a bonus up to +3, instead of the +2. It does not go above +3 because it is not supposed to go above 3.
You put Unarmored bonus +2 in your last post, are you saying that the character sheet is showing a benefit from unarmored? This should not be the case as you only use one calculation, either you are using an unarmored bonus, or you are wearing armor, you do not get the benefits of both.
I see what he is talking about. I tested and confirmed this is a bug.
The medium armor master feat caps AC bonus from dexterity to 3 when not wearing armor.
With my test character (18 dex):
No armor, no feat: 14 AC
No armor, medium armor master: 13 AC
Studded leather, medium armor master: 16 AC
The bug is that MAM caps Dex AC when unarmored, it works properly when wearing other armor types though.
Here is the share link so others can see the bug: https://ddb.ac/characters/4106614/zOPfGD (I do change this character from time to time to test for other bugs, so it may be wearing armor later)
I think this likely hasn't shown up previously because characters that wear medium armor and take Medium Armor Master are unlikely to have a DEX bonus higher than +3.
Thank you DxJxC for explaining what I meant! Only reason I noticed is I have a Barbarian in a campaign that was captured equipment removed dex of 18 and I have the feat. Admittedly a specific case but better to have everything 100% in my opinion!
Sorry I really should when trying to fix problems huh? In the "Add Items" section of equipment the magic item Orb of Shielding (Thunder) lacks the little triangle tag with an 'A' in it to show that it requires attunement. I added it to a character to see if it was just the tag that was missing, but it is missing the attunement option/requirement to attune, giving the +1 bonus automatically. Thanks again to DxJxC for the help!

This is fixed. Thanks.

If you choose the background house agent (Deneith) it gives you three proficiencies with game sets instead of one
if you get the medium armor master feat it won’t allow the dex bonus to go higher than +3 on any option even if you have higher
The background House Agent (Deneith) has been fixed.
The Medium Armor Master feat is working as intended.

Ah, understood.

Thanks for explaining in detail!

Found that the Orb of Shielding (Thunder) "doesn't require" attunement (doesn't allow)
What is the problem? Use more words.
