I think OP is referring to the Level 3 feature for Fey Wanderer, which is changes to the Ranger, rather than physical presents. Here are the ideas I've put down for my players in the past.
Shining/reflective unicorn horn.
Your shadow being replaced with an apparition which reflects the weather for the next 24 hours.
Your height changes approximately 1cm with every word you speak. Whether you grow or shrink is random, but cannot change your Size.
When you cast illusion or enchantment spells, the normal colors of those effects appear as purple, blue, and gold.
When you cast a Summon spell, the creatures seem to dance and cheer for you.
You grow a beard of clover. Each morning, roll a d4 to determine if there are flowers. 1)None 2) Red 3) Purple 4) White
Your skin gains the appearance of scales which shine in sunlight.
When you gain temporary hit points or recover HP, small fairies swirl around you momentarily.
Your hair moves on its own, even when there is no wind.
You leave ghostly colorful footprints which disappear after 7 seconds.
You grow cosmetic fur on various parts of your body. You determine where this fur manifests, and it grants no mechanical changes.
Your fingernails change every day to resemble normal brightly colored nails, claws, or talons.
Your hair tends to braid itself into new patterns each dawn, sometimes creating seemingly impossible configurations.
Your shadow is cast onto yourself, as if you and your shadow traded perspectives.
Mundane fires near you twitch and flutter as if fairies comprised the flames.
You gain a birthmark which other fey recognize as being fey in origin. You determine the location and appearance.
Your shirt, coat, or cloak always appear to be embroidered with flowers and small animals while you're wearing it.
Your left eye becomes golden in color, and appears to shine. Your right eye becomes black in color and appears to absorb light.
Any wooden weapon you hold is surrounded by tiny sprites. This effect ends when you release the weapon, unless you attach it to your person.
Your breath creates a small intangible fog when you speak, as if speaking in the cold, no matter the temperature.
