It should be setup to add additional backgrounds like you can languages and skills. I was awarded a second background after months of roleplay, spending 100 downtime,e days, and multiclassing to tie it altogether. Read up in the notes section on my character FinEllen Stouthawker ( they are the cliffnotes of several months of role playing).

I suggest that, in this case, you homebrew your own custom background, adding the features from both backgrounds, or you can just note it on the character and use the character sheet to add anything you gained through adding the new background.

This was what I thought when reading you got granted a second background.
DDB is set up to follow and allow what is described in the books and the books don't begin to consider multiple backgrounds (considering their description of backgrounds).
It would be a useful addition and hopefully a future sourcebook might allow for that potential. In our homebrew campaign, we have left our plane and have arrived in Ravnica and are being given guild associations as cover.

Guild associations/memberships are not backgrounds by default. You can be in a guild without having the guild specific background. You will get the guild spells though, those arent supposed to be tied to background, but that is how DDB shortcutted it.

If you've 'earned' an additional background, I'd suggest adding those features as a feat perhaps?
