Now I've found an error introduced by the DDB version.
There's a new monster called a Delerium* Dreg which has a 'basic' form and 11 variants. The DDB names are inconsistent, so while most are listed as "<Variant> Dreg", the "Bloated Delerium Dreg" and "Lambent Delerium Dreg" aren't abbreviated. Either would work, or to be more consistent with the book, they could be listed as "Delerium Dreg, <Variant>" so they'd all sort together. It invites confusion to have different naming styles mixed together.

Unfortunately, "lack of textual consistency in how X is handled" is a common complaint that has been raised here repeatedly regarding many different cases. Some of these cases are three or more years old at this point and have been raised many times over the years, and still have not been corrected on the site. As far as I know, many (most?) have never been acknowledged when raised.
I would be pleasantly surprised if these were corrected, but sadly I would not hold my breath.
Dungeons of Drakkenheim introduces a new condition called contamination and additional rules in Appendix C. We’re currently unable to support the effects of contamination such as mutations.
This section of the post refers to contamination as a condition - does that mean it's intended to show up under the conditions menu? I fully understand how showing the effects of various mutations would be impractical/impossible but given how central the contamination mechanic is to this setting, I was surprised by the complete absence of a contamination level tracker. Perhaps this is an error and not an intentional omission, though?
(I've managed fine with the custom feat I created as a workaround months ago and will continue to do so, but flagging this in case it's something that should be fixed.)
I'm not sure if this is a problem of the module's maps or the Maps feature itself but I'd very much like the grid overlay's opacity to be looked at. Some of the supplied maps have perfectly good grid that blends nicely (for example, st. vitruvio's cathedral) but on some maps the grid pops out so much it makes it difficult to look at and to make sense of the map itself (for example, castle drakken - main level).
In general, if the grid overlay is generated by DDB Maps feature, it would be nice to have an opacity slider (and maybe even grid color picker to boot) that could be adjusted to everyone personal liking.
In the Pale Man's stat block, the DDB version is missing his known cantrips, which is tied to one of his legendary actions. The book version has them listed as:
chill touch, mage hand, mending, prestidigitation, and shocking grasp.

The description page for the Comet Smasher magic weapon has links that direct to the wrong items. All of the links to the various versions of it link to the Hellfire Weapons instead of the Comet Smasher weapons. The pages for the various Comet Smasher items can still be found through the search bar at the top of the website though, so they still exist, just have to go through an extra step or two.
Ah! Thank you! Does that mean The Book of Many Things is 3rd party, too? I gave one of my players an item from this book but they can't add it in DnDBeyond and I can't buy it individually.
Okay, I have an odd one and it seems to be working it's way into other source material. I can't search for or find Delirium forged or other magic items in either the magic items field on the main page. Links work, Tool Tip stuff works, character sheets can find them in inventory. But I can't just go to the item page directly. I have to go to the source book, find the item.

When searching stuff like magic items you need to go to advanced filters then in the partnered content select yes. Then you can search.

The Scepter of Saint Vitruvio isn't showing up as a magic item. Have Drakkenheim content enabled.

I posted this a little while ago but the Delerium Crystal Focus wrongfully gives the user a +2 to their saving throws instead of their spell save DC.
