Very niche as I don't think many people even noticed it exists but the Boomerang Shield has an attack action and damage but doesn't appear in the Actions when equipped and attuned. One of my players is living out his Captain America fantasies and seem stupid to have to home brew an item that actually exists
You could also try creating a custom action directly on the character sheet, if you go to the Actions tab and hit Manage Custom in the top-right corner.

Seems like this is still an issue! I just created it as a custom action, but it's annoyingly in my 'Other' section and not with my weapons.
