I tried to create from scratch a pair of gloves that give +1 bonus to dexterity checks. It is declared as a Wondrous Item FWIW. It has one modifier:
Modifier type: Bonus
Subtype: Ability Checks
Ability Score: Dex
Fixed Value: 1
I've tried with nothing entered under Dice Count and Die Type, and I've tried with 0d4, but either way when I add it to my character's equipment and check the checkbox to equip it, it seems to add a my dexterity bonus to every proficiency in the list.
I assume this is a bug? If this is how it's designed, is it even possible to create the simple item I'm trying to create?

It's not a bug, it's just that this particular combination of modifiers isn't intended to do what you're trying to do. When you select "Ability Checks" as the subtype, you're saying you want the item to provide a bonus to all ability checks (like a Luckstone). The Ability Score selection is to select an ability score whose modifier will be used to determine the amount of the bonus.
None of this stuff is very well documented, so it's a very easy mistake to make.
To do what you're trying to do, use the method spamandtuna recommended.

I’d try changing the Subtype to “Dexterity Ability Checks” and unselecting Ability Score.
Note that homebrew edits can sometimes take up to 15 minutes to appear on your sheet.
