Hex Warrior feat grants proficiency with medium armour, shields, and martial weapons. Yet, even with a +2 dex mod using scale mail my PCs AC is only 16. Bug?
Update: determined that proficiency bonus is not being added to total AC. Same result woth studded leather resulted in a 14 AC with the same dex mod of +2

Hexblade Warrior's feat also ignored in relation to apply charisma mod to weapons you're proficient with instead of Str/Dex, even with pact of the blade as well as proficiency bonus
Even with the feature you don't add your proficiency bonus to your AC (or ever afaik) just because you are proficient with a type of armor. The 16 AC is correct.
EDIT: Proficiency with armor never adds your proficiency bonus to your current AC regardless of level unless a feature specifically states is increases your AC total by a set amount. Having proficiency with a type of armor allows you to wear it without being imposed with disadvantage on any ability check, save, or attack roll that involves Strength or Dexterity. You can also not cast spells iirc if you are wearing armor which you are not proficient with.

Scale Mail is 14 + Dex modifier (max 2) so I doubt it is a bug.

As I said Hex Warrior class feat grants proficiency in medium armour which means total AC should be including a proficiency bonus of +3 at level eight
So 14 + 2= 16 + 3 = 18
