The table of contents does not track properly with the text as it is read and the major sections collapse in a weird manner once you get to Fields of the Dead. I think part of the issue is that High Forest is reading as a major section rather than a minor section under Independent Realms and this seems to be throwing everything off.
Actually, the issues happen further up as well and The Moonshaes also seems to be a major section when it should be a minor section under Island Kingdoms.
