I wonder if it might be left to the DM's discretion as to whether or not you could have a case of full blown death from reaching 6 levels of exhaustion. I think it's set up so that, generally, the limitations imposed from 4 out 5 levels of exhaustion might seemingly make it impossible to push a character any further to die from exhaustion, but it's also possible that if a character we're getting exhaustion levels from multiple sources, they could end up with insta-death without taking any damage, and without making any death saves - and it sense to me that 5e has made efforts to get rid of insta-death.
Is it possible this is something that's been revised in different printings of the rules because they didn't want to have a loop hole in SRD? That could be a problematic and exploitable loop hole to have for competitive PvP tournament play.

Under the Appendices/Conditions/Exhaustion, there's a discrepancy between this site and the SRD.

There's no text explicitly stating what happens if you have more than 5 levels of Exhaustion (ie the character dies), so this seems to be an omission?

The 6th level of exhaustion is in the PHB as well (last printing).
